Application Process:

To file an application for donor travel to the transplant center, the transplant professional must login to the NLDAC website and select “New Application”. We recommend using the Donor and Recipient Application Worksheet and Attestation Forms shown below to gather the data needed to complete an application. Because eligibility is based on yearly household income of the recipient and the donor, a copy of the income documents used to file the application must be kept on file at the transplant center for future audit. Examples of accepted income documents are: Federal tax return, pay stubs,
W-2, Social Security Statements, Medicaid, HUD or other documents. If you need assistance, call 703-414-1600 or toll free at 888-870-5002. We are happy to help.

NLDAC cannot reimburse expenses that have already occurred. We recommend complete applications with Preference Category 1, 2 and 3 be submitted as early as 6-8 weeks, but no later than 10 business days before the surgical procedure or travel. Complete applications with Preference Category 4 may also be submitted as early as 6-8 weeks, but no later than 15 business days before the surgical procedure or travel. If the recipient or donor household income is greater than 300% of the HHS Poverty Guidelines, you may request a waiver for financial hardship. Please complete the Financial Hardship Waiver Worksheet and attach to step four of the web-based application or fax to 703-414-7874.
