How to Submit Applications (for Transplant Professionals)

  1. Register as an application filer here, if you have not already done so.
  2. Use the eligibility screening tool to make sure the donor and recipient are eligible for the program. If the recipient’s household income is greater than 350% of the Federal Poverty Guidelines, they will need to submit a financial hardship waiver worksheet and demonstrate qualifying expenses that bring their household income within the guidelines. These applications are classified as category 4, and require approval from HRSA.
  3. Gather the application worksheets, attestation forms, and income verification documents from the donor and recipient. Review all the information and sign the attestation form. Calculate the donor and recipient household income based on the documentation they provide. Log on to our website, create a new application, and transfer the information from the worksheets to the application. You will enter the household income information on step 4. You do not need to upload the household income documents, but you do need to attest that you have calculated the household income based on the documents and will keep them on file for 6 years.
  4. If the donor is applying for reimbursement of lost wages, ensure you have received their signed W-9 and two most recent pay stubs. If they are self-employed, they can submit a federal income tax return with schedule C, IRS Form 1099, or income summary generated by the company for which they do contract work (like Uber or Instacart). You will upload these documents on step 3a. 
  5. When you have entered all the information, click the 'submit' button for each type of reimbursement the donor is requesting (travel, lost wages, and dependent care).
  6. Category 1 and 3 applications must be submitted at least 10 business days before travel. Category 4 applications must be submitted at least 15 business days before travel. In case of a medical emergency that prevents a prospective liver recipient from waiting 10-15 business days for a transplant, check the box to indicate that it is an urgent liver donation. These applications can be approved in 1-2 business days. Applications cannot be rushed for any other reason.
  7. File the donor and recipient attestation forms and household income documents in their respective medical records in case of audit.
